Congratulations for making this important effort to plan for the future care, support and love of your pet. PetGuardian's goal is to offer you an affordable and comprehensive solution. We encourage you to consult with your attorney to incorporate your PetGuardian Pet Trust Plan documents into any existing estate plans in addition to appropriately establishing a separate pet trust fund.
The PetGuardian pet trust plan costs only $500; changes to the forms are free when completed online with only a nominal charge for changes submitted through the mail. 
1. Complete the PetGuardian Pet Trust application
Complete PetGuardian's questionnaire which provides sample answers and saves your information as you go. You can skip sections and use the links on the left side of the page to navigate through the application process. You can always save your information, log off, return to the web site, log on using your user ID and password, and continue to complete the application when it's most convenient for you.
2. Print your documents
After your payment is processed you have the option to print all of your documents together or each document set separately. The documents you will receive: customized pet trust document, comprehensive care instructions, cost analysis and our letter of introduction. You can also choose to have PetGuardian print and mail the documents to you at no additional cost.
You will receive confirmation of your order through both email and regular mail.
Included in your email confirmation will be instructions outlining how you can easily make changes to your documents online at no additional charge. You can expect to receive a final confirmation in the mail that will include emergency ID wallet cards and pet alert cards.

If you are not comfortable going through the process online, please feel free to download the application and mail it to us when you're finished. If you have more than two pets, you will need to download this additional form.
